When contacting our office, please provide your name and date of birth so we may efficiently locate your medical records.
For medical concerns or questions please contact our office during normal business hours. Our nursing staff returns patient calls and phones in prescriptions after 4:00 p.m. daily. If you have an urgent need that can not wait until the following business day, you may call (205) 280-1010 and listen to the recorded message to obtain the current on-call pager number. If you have an emergency you may go to the nearest emergency room or call 911.
When calling about a non-medical question (such as billing or insurance), please call us during normal business hours and ask to speak with the appropriate office staff to discuss your issue. Please do not ask to speak with a physician regarding these matters, as the office staff is better equipped to handle these questions.
We require that all patients update any information that we have on file at the time of your arrival. You will be asked to update your records each year. We encourage patients to provide us with an updated copy of their medical history and a list of all medications they are currently taking. We recommend that you ask your previous primary care physician to mail or fax your medical history to us prior to your appointment. Please notify us of any urgent needs when you call and we will arrange for you to see a Physician immediately. After seeing your Chilton Family Medicine Physician, please visit the front desk to schedule your next appointment. If you are unable to keep your appointment, we would appreciate your notifying us at least 24 hours in advance.
